Job Summary: The chief responsibility of the Young Adult Resident Service Coordinator is to provide direct, supportive, individualized care in a positive and holistic way to young adults moving towards independence within an LGBTQ affirming environment. The Young Adult Resident Service Coordinator will support young adults, many of whom identify as LGBTQIA+) in the retention of housing, employment, the continued development of skills and participation in activities that will enhance their ability to become self-sufficient. The services also include keeping accurate records in a computerized system.
Essential Duties and Responsibilities
· Community Engagement:
o Identify, connect and engage with organizations within Young Adult Continuum of Care to build network of potential applicants for young adult units
o Provide assistance in navigating application process to Project HOME units and/or more immediate, appropriate community supports
o Co-Facilitate housing prep, tenancy skills seminars and life skills classes
o Provide oversight of the lease up process including reviewing applications, processing paperwork and conducting interviews
o Provide discharge planning and linkage to the Alumni Program prior to discharge
o Assist young adults with coordination of benefits and resources
o Support young adults in filing for benefits or resolving any issues through DPW, SSA, and unemployment.
o Assist young adults in job search process.
o Assist young adults in coordination of their education goals, including GED testing, college application and financial aid.
o Assist young adults with medical and psychiatric referrals, communication and escorting residents to appointments, as needed.
o Support young adults in maintaining a commitment to recovery, including conducting random urine screens as necessary.
o Understand, anticipate, and respond to the issues, expectations, and special needs that each young adult may have.
o Work with young adults individually as needed, including escorts (e.g. one to one counseling, providing an escort to appointments, interviews and activities)
o Work with young adults to assess needed resources and facilitate connection to resources in the community; monitor client’s participation in activities.
o Maintain case records, as required in a timely and thorough manner including case notes, goal plans and assessments as required.
o Utilize database for all documentation.
· Meet as scheduled with the following people:
o Participate in monthly staff & resident meetings and special committees.
o Meet weekly in supervision with Program Manager and attend team meetings.
o Participate in trainings and continuing education opportunities.
o Facilitate Young Adult Life Skills classes (individually and in a group setting)
· Alternate on call with other residential staff for emergencies:
o Be flexible to meet the changing needs of the young adults we serve and be available by phone
o Work with young adult and supports through crises, assessing the situation and making needed plans/interventions.
o Become familiar with all emergency procedures.
· Experience with LGBTQ identified homeless youth, youth at risk of being homeless, as well as youth aging out of foster care with limited supports and resources.
· Good verbal and written communication, organizational and interpersonal skills. Team experience. Some computer literacy. CPS, BSW or related degree plus one year experience.
· Valid PA Driver’s License
· Experience in brief counseling, assessment and judgment, crisis management skills, trauma informed practice and resiliency.
· Working knowledge of LGBTQ youth services in Philadelphia.
· Ability to handle physically and emotionally challenging situations.
· Master’s Degree
